Program Exclusions & Limitations
For complete details, see Plan Terms and Conditions on our website. Youth plan is intended for ages 13 and younger. If periodontal infection is present, the member may be required to upgrade to the Perio plan. Monthly payment option will incur a 20% surcharge and a $99 enrollment fee.
